Transition Plus with door

Transition Plus Services (TPS) is a program designed to provide an adult environment for 18-21 year old young adults with disabilities and identified transition needs. A transition focused plan will be developed in the high school prior to coming to Transition Plus. This plan should include appropriate services, resources and agency linkages, which will assist with providing a smooth transition from high school to the adult world.

Through programming in the classroom, in the community, and on the job, students are exposed to all areas of adult life. Related services are available as determined by the IEP team.

A referral to the program can be made through Minneapolis Public Schools High School IEP Teams, our intake coordinator, the placement center, or other agencies. A complete referral packet along with a copy of the student's most recent due process paperwork will be needed to plan appropriate services. Typically students have completed four years in a high school setting prior to attending TPS. Of course, there are exceptions which will be reviewed by the IEP team, the intake team, and the Administrator.
