Transition Plus Services (TPS) is designed to provide an adult environment for students with disabilities, ages 18-21, with identified transition needs. A transition focused plan will be developed to provide appropriate services, resources, agency linkages, and to provide a smooth transition from high school to the adult world.
Through programming in the classroom, in the community and on the job, students are exposed to all areas of adult life. Related services are available as determined by the IEP team.
A referral to the program can be made through Minneapolis Public Schools High School IEP Teams, returning students, the placement center, or other agencies. A complete referral packet along with a copy of the student's most recent due process paperwork. Typically students have completed four years in a high school setting prior to attending TPS. Exceptions to this are at the discretion of the IEP team, Transition Plus Teams, and Administration.
